LANGLEY COMMUNITY CHORUS
LIABILITY WAIVER FOR Spring 2023
As we transition back to in-person chorus rehearsals, the health and safety of our members remain a top priority. We are all eager to resume the normal activities of a choral season but in order to do that we are requiring our members to be fully vaccinated in order to participate in in-person activities with the Chorus. We also ask our members to acknowledge the risks inherent in rehearsing and socializing together and to take responsibility for their choices with regard to participation in in-person chorus activities.
Langley Community Chorus Members are expected to:
•be fully vaccinated for Coronavirus/COVID-19 and are 2 weeks beyond their final dose of the vaccine. (proof of vaccination required)
• Abide by the protocols set out in the Health and Safety Guideline document
• Not attend rehearsals or performances if experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
• Refrain from attending rehearsals or performances for 5 days if they believe that they have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
• Refrain from attending rehearsals or performances for 10 days or longer if they themselves have been diagnosed with COVID-19 and no longer have symptoms.
• Follow all BCCDC recommended guidelines as much as possible and limit their exposure to the Coronavirus/COVID-19.
• Understand and agree to the requirements of the LCC Covid-19 policy for wearing a mask while singing.
• Understand and agree to the requirements of the LCC COVID-19 policy for social distancing within the rehearsal or performance venue.
ACKNOWLEDGEMENT:
I voluntarily seek to participate in LCC’s rehearsals and performances for the Spring Season 2022 and acknowledge that I may be increasing my risk of exposure to the Coronavirus/COVID-19 in so doing.
I acknowledge that I must comply with all set procedures to reduce the spread while attending rehearsals and performances. I acknowledge that the Langley Community Chorus (LCC) cannot guarantee the safety of the activities in which I am participating or that I will not become infected with the Coronavirus/Covid-19, despite the safety measures taken by the LCC. I understand that I am responsible for assessing my own risks in participating in the LCC.
I have read and agree to the Langley Community Chorus’ Health and Safety Policy and I hereby release and agree to hold Langley Community Chorus harmless from, and waive any liability or claim that I or anyone on my behalf may have against the LCC with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection with LCC rehearsals and performances. This liability waiver and release extends to the LCC together with all Board members, singers, accompanist and Artistic Director.
